Enrolling in the Plan
You may enroll yourself and your eligible dependents
in the Dental Plan.
Employees
The chart below includes information on how to enroll
in the Plan under various circumstances and how your coverage is
affected.

Spouse, Domestic Partner, and Children
The chart below includes information on enrolling
your spouse, domestic partner, and children in the plan.

If You and Your
Spouse/Domestic Partner Both Work for the Board
You cannot be covered as both an employee and a dependent
in the Dental Plan.
If you and your spouse/domestic partner both work for the Board and
are eligible for dental coverage, one of you (but not both) may
enroll your dependents in the plan. If you do not have any children,
or your children are not eligible, you and your spouse may each
choose "self-only" coverage.
Coverage Levels
The Dental Plan offers two coverage levels from which
to choose. You may elect:
- Self-only coverage
- Family coverage
If you choose family coverage, all of your eligible
dependents will be enrolled in your Dental Plan coverage. Add the
names of your new dependents to your family coverage within 60 days.
