Your Dental Insurance

When Coverage Ends

Your coverage ends on the earliest of the following:

  • The day you are no longer eligible for plan coverage
  • The date you stop paying required contributions
  • The last day of the pay period in which your Board employment ends
  • The date the plan is discontinued.

Your dependent's coverage ends on the earliest of the following:

  • The date your coverage ends
  • 31 days after the end of the month in which your dependent no longer qualifies for coverage.

   Extended Coverage  

Dental Plan benefits will be payable for the following services and supplies if received within three months after plan coverage would otherwise end:

  • Bridge pontics and similar services for which impressions were taken while coverage was in effect
  • Endodontic services (such as root canal therapy) for which teeth were opened while coverage was in effect
  • Crown, inlay, or onlay for which the tooth was prepared while coverage was in effect.

All other services are considered separately on the date the service is provided.
