Employee Information Forms

We need some basic information to prepare for your first day of employment. The information collected is used to set up your pay, computer access, and personnel record.

To view and print the Board's forms, you'll need to have Adobe Acrobat Reader installed on your computer, which you can obtain free of charge from the Adobe Website.

Please fill out these forms and bring them with you on your first day. (The forms listed below are for full-time employees. Forms for paid interns and unpaid interns.)

During orientation, you will receive information on enrolling in the Board's core benefits programs (health, dental, vision, life insurance, and Federal Reserve System Thrift Plan).

Required for your first day

Please bring the following information with you on your first day:

  • Valid driver's license and a social security card or birth certificate or a valid United States passport.
  • Your home address (P.O. Box not acceptable for tax purposes)
  • A copy of your checking account/savings account number and your financial institution's nine-digit ABA number (i.e., voided check), or open an account with the FRB Federal Credit Union. (The Board pays by direct deposit only.)
  • Official transcript from college or university, if not previously submitted.

To make sure that we have time to answer any questions you may have about the Board and our benefits programs, please be sure that you complete the following forms and have them ready to submit to the orientation facilitator on your first day.

    Please read the instructions before completing this form and review the last page for the list of acceptable documents that you will need to bring with you to establish identity and employment authorization.

  • Background Investigation and PIV cards
    Prior to your start date, the Board's Personnel Security section will forward a link to the Electronic Questionnaires for Investigations Processing System (e-QIP), a web-based automated system that has been developed for the U.S. Office of Personnel Management (OPM), Federal Investigative Services Division. The questionnaire is used to conduct your suitability/security investigation for employment here at the Federal Reserve. Once your investigation is complete and favorably adjudicated, you will be issued a Personnel Identity Verification (PIV) card, your employee ID card that will allow you access to Federal Reserve Board and most government buildings. This process takes several weeks. You will be provided with a temporary identification card that you can use until your PIV card is issued. A Federal Reserve staff member will contact you when your card is available. Remember that Federal Reserve employees are required to wear the temporary identification or PIV card at all times.

    Information on employees at the Board with disabilities are used to prepare required reports for the Equal Employment Opportunity Commission, to justify employment of or to support personnel and purchase of special equipment, and to plan building renovations.
